Responsibilities

• Provide and direct patient rehabilitation care within accepted practices.
• Evaluate patient needs and establish treatment plans.
• Coordinate supervision of treatment plans with staff and other disciplines.
• Screen patients according to directed schedules and practices.
• Document and report clinical and rehabilitation issues to the Team Leader.
• Participate in facility processes and services as directed.

Education
