Touchmark on West Prospect in Appleton, Wisconsin, is seeking a compassionate Registered Nurse (RN) to join our Health Services team in a full-time capacity. You will provide direct nursing care, assess resident health, and coordinate medications and care plans within an Assisted Living and Memory Care environment.

Ideal candidates hold an active Wisconsin RN license, with 1–2 years of experience with older adults, CPR/AED and First Aid certifications, and strong leadership and communication

Qualifications

• Current, unencumbered Wisconsin RN license.
• 1–2 years of experience with older adults preferred.
• CPR/AED and First Aid certification per state requirements.
• Strong assessment, documentation, and communication skills.
• Ability to lead, train, and delegate effectively.

Responsibilities

• Provide direct nursing care within RN scope of practice.
• Assess resident health status and develop individualized service plans.
• Administer medications and treatments using eMAR.
• Oversee and coordinate medication aides and clinical team members.
• Serve as a primary point of contact for provider communication.
• Ensure compliance with state licensure and regulatory standards.
• Document assessments, observations, and care accurately.
• Support quality improvement and clinical excellence initiatives.
