Touchmark on West Prospect in Appleton, WI, seeks an experienced RN to serve as Clinical Care Manager. This exempt, full-time role combines bedside nursing with leadership to oversee Assisted Living and Memory Care services.

The RN will assess residents, coordinate care with physicians and families, and mentor nursing staff to ensure high-quality, person-centered care and regulatory compliance.

Qualifications

• Current, unencumbered Wisconsin Registered Nurse license.
• Four+ years nursing experience, preferably with older adults in senior living, memory care, or similar.
• Previous leadership, clinical oversight, or supervisory experience is preferred.
• Strong resident assessment and clinical decision-making skills.
• Knowledge of medication management and senior care practices.

Responsibilities

• Provide clinical leadership and oversight to ensure high-quality, person-centered care.
• Assess residents and monitor health status with documentation and follow-up.
• Develop and update resident service plans based on clinical needs.
• Coordinate resident care with physicians, families, pharmacies, and health partners.
• Oversee medication management and ensure orders and service plans are followed.
