Job summary

Pine Haven Christian Communities is seeking a compassionate Social Services Coordinator to support the emotional and social well-being of residents and their families. You will assist with psycho-social assessments, coordinate admissions, and connect families with community resources.

You will collaborate with staff and outside providers to ensure high-quality care, participate in care planning, and contribute to QA efforts while upholding Pine Haven’s mission and values.

Qualifications

• Prior social services experience in a senior living setting required.
• Ability to make independent decisions when circumstances warrant such action.
• Ability to deal professionally with residents, families, staff, and external agencies.
• Patience, tact, cheerful disposition and enthusiasm while supporting residents.

Responsibilities

• Assist psycho-social assessments for potential residents.
• Provide tours to potential residents and families.
• Coordinate admissions of new residents.
• Support emotional and social needs of residents and families.
• Meet with residents and families regarding emotional, social, or physical needs.
• Coordinate with providers for on-site visits (podiatrist, audiologist, etc.).
• Develop social services programs to enhance care for residents and families.
• Serve on the Administrative On Call Rotation.
• Implement QA initiatives and ensure infection control.
