Job summary

Pine Haven Christian Communities is seeking a compassionate Social Services Coordinator to support the emotional and social well-being of residents and their families. You will assist with psycho-social assessments, coordinate admissions, and connect families with community resources.

You will collaborate with staff and outside providers to ensure high-quality care, participate in care planning, and contribute to QA efforts while upholding Pine Haven’s mission and values.

Qualifications

• Prior social services experience in a senior living setting required.
• Ability to make independent decisions when circumstances warrant such action.
• Ability to deal professionally with residents, families, staff, and external agencies.

Join the Pine Haven family! We are looking for a compassionate and experienced Social Services Coordinator to support the emotional and social well-being of our residents and their families. This position plays an important role in helping residents and ensuring they receive the support, resources, and quality care they deserve.

• Assist with psycho-social aspect of potential resident assessments
• Provide tours to potential residents and families
• Coordinate, and complete the admissions of new residents
• Responsible for providing residents and families with the social and emotional aspects of transitioning to our facility
• Meet with residents and families regarding emotional, social, or physical needs
• Assist staff with challenging resident or family situations
• Connect residents and families with community and government benefit programs and resources
• Coordinate, facilitate and participate in service plans, and complete follow up tasks from the care planning
• Investigate, document, and follow-up on resident rights concerns, including lost items
• Develop social services programs to provide higher level of care for residents, families, and staff
• Coordinate schedules for residents and outside providers who come on-site, such as podiatrist, audiologist, ophthalmologist/optometrist
• Work closely with support department staff and managers, including providing direction as needed to staff and sharing concerns with managers
• Serve on the Administrative On Call Rotation
• Provide Quality Assurance structure, review of programs, and implement QA initiatives as they are developed
• Follow infection control and universal precautions policies and procedures to ensure that a sanitary environment is always maintained
• Attend and participate in required training, conducted at time of hire and/or annually, that will include training designed to assure resident safety: i.e. fire safety, missing resident, evacuations, disaster response, etc.
• Report any observed changes in a resident’s needs and/or condition immediately to a license nurse or manager as appropriate
