BAYADA Home Health Care, Inc. seeks a qualified Physical Therapist (PT) for Senior Living Visits in New Jersey. This full-time role offers one-on-one client care across Gloucester, Cumberland, and Salem counties in facility settings.

You will evaluate mobility and function, plan individualized care, and provide therapy focused on improving independence, with support from BAYADA’s interdisciplinary team and EMR tools.

Qualificações

• Current New Jersey Physical Therapist (PT) license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of recent experience as a Physical Therapist.
• Home health or senior living experience preferred, but not required.
• Strong clinical assessment, communication, interpersonal, and organizational skills.
• Ability to work independently and effectively manage time and schedule.
• Prior experience with electronic medical records (EMR) preferred.
• Reliable transportation and ability to travel to facilities throughout Gloucester, Cumberland, and Salem counties.

Responsabilidades

• Provide one-on-one physical therapy visits to clients in facilities.
• Perform comprehensive physical therapy evaluations, including mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Develop and implement individualized plans of care based on client needs, goals, and physician orders.
• Provide skilled physical therapy interventions, including therapeutic exercise, gait and balance training, mobility training, transfer training, and other appropriate treatments.
• Help clients improve or maintain strength, mobility, safety, and functional independence.
• Monitor client progress and adjust treatment plans as appropriate.
• Educate clients, families, caregivers, and facility staff on therapeutic exercises, mobility techniques, fall prevention, and safety strategies.
• Collaborate with nurses, therapists, physicians, facility staff, and other members of the interdisciplinary care team.
• Complete timely and accurate clinical documentation using BAYADA’s electronic medical record (EMR) system.
