Job summary

BAYADA Home Health Care is seeking an experienced Physical Therapist to join our Senior Living office for full-time, one-on-one visits to adults in facilities across Gloucester, Cumberland, and Salem Counties, NJ.

The role emphasizes autonomy, comprehensive PT evaluations, and individualized plans of care, with collaboration across an interdisciplinary team and EMR documentation support.

Qualifications

• Current New Jersey Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Minimum of one year of PT experience.
• Home health or senior living experience preferred but not required.

Responsibilities

• Provide one-on-one physical therapy visits to clients in facilities.
• Perform comprehensive physical therapy evaluations, including mobility, strength, balance, gait, endurance, transfers, safety, and functional abilities.
• Develop and implement individualized plans of care based on client needs, goals, and physician orders.
• Provide skilled physical therapy interventions, including therapeutic exercise, gait and balance training, mobility training, transfer training, and other appropriate treatments.
• Help clients improve or maintain strength, mobility, safety, and functional independence.
• Monitor client progress and adjust treatment plans as appropriate.
• Educate clients, families, caregivers, and facility staff on therapeutic exercises, mobility techniques, fall prevention, and safety strategies.
• Collaborate with nurses, therapists, physicians, facility staff, and other members of the interdisciplinary care team.
• Complete timely and accurate clinical documentation using BAYADA's EMR system.
