• Perform comprehensive physical therapy evaluations and establish individualized, evidence-based plans of care across adult/geriatric physical therapy domains.
• Deliver skilled treatment interventions addressing strength, balance, gait training, transfer mechanics, functional mobility, endurance, joint integrity, and daily routines.
• Target fall prevention, balance recovery, mobility device prescription and training, apartment safety, and environmental navigation.
• Educate residents, families, caregivers, and staff on functional independence, fall risk reduction, safe transfers, and mobility support.
• Collaborate with interdisciplinary team partners, physicians, community leadership, wellness directors, life enrichment teams, and caregivers.
