Job summary

Glenbridge Health and Rehabilitation is seeking a skilled Physical Therapist to join our Boone, NC team on a PRN basis. The PT will evaluate residents, design personalized plans, and provide therapeutic interventions to improve independence and function.

Responsibilities include documenting progress, collaborating with an interdisciplinary team, and educating residents and families on therapy programs. CPR certification and licensure are required, with travel as needed.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Current and valid Physical Therapist license.
• CPR certification required (or obtainable upon hire).
• Prior experience in skilled nursing or long-term care preferred.
• Strong communication, clinical, and organizational skills.

Responsibilities

• Conduct comprehensive physical therapy evaluations and develop personalized care plans.
• Deliver physical therapy treatments, including therapeutic exercises, gait training, balance activities, and functional mobility.
• Monitor and document residents’ progress and adjust plans as needed.
• Collaborate with nursing, occupational therapy, speech therapy, and physicians for holistic care.
• Educate residents, families, and caregivers on therapy programs and safety measures.
