Job summary

Prava Therapy in Raleigh, North Carolina is seeking a full-time Physical Therapist to join our luxury, resort-style senior living community. You will lead comprehensive evaluations and design evidence-based plans of care for adult and geriatric residents.

Collaborate with physicians, wellness directors, and caregivers to promote mobility, fall prevention, and independence, while guiding residents to stay active and engaged in daily life.

Qualifications

• Proven ability to evaluate adult/geriatric clients and develop care plans.
• Strong collaboration with interdisciplinary teams.
• Commitment to fall prevention and functional mobility.

Responsibilities

• Perform comprehensive physical therapy evaluations and establish individualized, evidence-based plans of care for adult/geriatric residents.
• Deliver skilled treatment addressing strength, balance, gait, transfers, functional mobility, and endurance.
• Target fall prevention, mobility training, and safe navigation of the community environment.
• Educate residents, families, caregivers, and staff on independence and safety in daily activities.
• Collaborate with physicians, wellness directors, life enrichment teams, and caregivers.
• Monitor progress, document evaluations and treatment sessions, and adjust plans based on outcomes.
