TELOS Health Systems, Inc. is seeking a licensed Physical Therapy Assistant (PTA) to join Sequoia Care in Deland, FL, delivering outpatient therapy in a senior living setting. This PRN role focuses on hands-on rehabilitation under a licensed PT, with flexible scheduling to fit patient needs.

You will implement treatment plans, monitor progress, educate residents and caregivers, and maintain accurate documentation in compliance with state and federal guidelines. Florida PTA license is required.

Qualifications

• Associate degree from an accredited PTA program.
• Current Florida PTA license.
• Senior living or similar experience preferred.
• CPR certification and strong documentation skills.

Responsibilities

• Implement physical therapy treatment plans as directed by a Physical Therapist.
• Monitor and document resident progress and response to treatment.
• Educate residents and caregivers on exercises and safe movement techniques.
• Communicate with interdisciplinary teams to ensure coordinated care.
• Maintain accurate clinical records in compliance with Florida state and federal guidelines.
