TELOS Health Systems in Altamonte Springs, FL is seeking a part-time Outpatient Physical Therapist to join Sequoia Care. You will assess, develop, and implement personalized treatment plans to help residents regain movement, reduce pain, and restore independence in a senior living setting.

Qualifications

• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• Current Physical Therapist license in Florida.
• Senior living or similar experience preferred.
• CPR certification as required by facility or state.
• Strong clinical documentation and communication skills.
• A passion for resident-centered care and improving quality of life.

Responsibilities

• Perform evaluations and develop individualized treatment plans
• Deliver skilled physical therapy services to residents in senior living setting
• Monitor and document resident progress, adjusting plans as needed
• Educate residents, caregivers, and staff on mobility, exercise, and prevention strategies
• Collaborate with interdisciplinary teams to ensure coordinated, high-quality care
• Ensure compliance with Florida licensure and documentation standards

Education
