Job summary

Qualifications

• Licensed Physical Therapist in Massachusetts

Responsibilities

• Make outpatient visits to clients in designated senior living communities.
• Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, ambulation patterns, strength, endurance, balance, and safety assessments.
• Develop and implement individualized care plans, including manual therapy, gait training, balance, and other interventions.
• Continually assess and revise the plan of care and participate in multidisciplinary care plan meetings.
• Educate clients, family members, or staff in therapy services to promote health, safety, and independence.
• Accurately document observations, interventions, and evaluations in the client record.
