BAYADA Home Health Care is seeking an experienced Physical Therapist for a full-time outpatient home health role serving Londonderry, NH and surrounding Dover, Rye, Exeter areas. The position requires a NH PT license and at least one year of clinical PT experience, with EMR documentation and independent time management essential.

Qualifications

• NH PT license required.
• Minimum 1 year PT experience.
• Graduate from APTA/AMA-accredited program.

Responsabilités

• Make outpatient home health visits in designated territories.
• Perform mobility, gait, balance and safety assessments.
• Develop and implement individualized PT care plans.
• Collaborate with nursing on multidisciplinary plans.
• Educate clients and families on rehabilitative care.
• Document observations and interventions using EMR.
