BAYADA Home Health Care, Inc. in Easton, MA is seeking a Physical Therapist for a full-time or part-time role providing home health visits and outpatient therapy within a senior living community. • Current Massachusetts Physical Therapist license.
• Outpatient experience a plus but not required.

Responsabilidades

• Make outpatient visits to clients in designated senior living communities.
• Perform diagnostic tests and measurements related to mobility, strength, balance, and safety.
• Develop and implement individualized care plans including manual therapy, gait training, and balance interventions.
• Continually assess and revise the PT plan of care with the multidisciplinary team.
• Educate clients and family members on therapy services to promote health, safety, and independence.
• Accurately document observations and interventions in the chart via touchpad tablet.
