Pine Park Health, Inc. is seeking a full-time Nurse Practitioner for field-based primary care across the SF Peninsula. You’ll serve older adults in senior living communities, with a focus on building relationships, care coordination, and personalized care plans.

You’ll travel between partner communities and have Fridays remote for documentation and development. The role offers a total target cash of $150,000–$190,000 (base + bonus) with equity and a comprehensive benefits package including PTO,

Qualifications

• Active, unrestricted California NP license with Furnishing Number.
• Interest in medically complex older adults and preventive primary care.
• Willingness to provide onsite care across the SF Peninsula, including driving between partner communities.
• Adaptability in a growing, evolving care model.
• Strong collaboration and communication with clinical team.

Responsibilities

• Provide comprehensive primary care to older adults in senior living communities.
• Develop and manage individualized care plans reflecting medical needs and patient goals.
• Deliver CCM visits and complete documentation.
• Collaborate with RNs, MAs, care navigators, and community staff to coordinate care.
• Lead clinical huddles to review patient needs and coordinate follow-up.
• Partner with community leadership to implement care plans in living environments.
• Collaborate with Medical Director to improve workflows and care standards.
