Pine Park Health Management Inc. is hiring a Full-Time Nurse Practitioner to deliver care in the East Bay San Francisco market (Oakland, Walnut Creek, Concord).

The role focuses on a smaller, complex patient panel within a value-based care model, supported by a multidisciplinary team to optimize patient outcomes. The position emphasizes person-centered care, onsite visits across Senior Living Communities, and collaboration with care navigators to maximize provider time and impact.

Qualifications

• Licensed Nurse Practitioner with geriatrics experience.
• Comfortable in a fast-paced, changing environment.
• Willingness to provide onsite clinical across the East Bay San Francisco region.

Responsibilities

• Provide onsite primary care to older adults living in Senior Living Communities.
• Develop and execute detailed person-centered care plans for primary care patients.
• Collaborate with Medical Director to develop policies addressing day-to-day practice and guidelines.
• Conduct monthly Chronic Care Management visits and review CCM documentation.
• Lead regular clinical team huddles and provide leadership.
