Qualifications

• Current CMT certification in the state of practice.
• High-school diploma or GED.
• Prior direct care experience preferred.
• Ability to work as part of an interdisciplinary team.

Responsibilities

• Prepares and administers medication to residents under nursing supervision and documents.
• Provides medical and physical care to residents to meet daily needs.
• Assists with personal care needs and supports resident independence.
• Reports changes in resident condition to nursing staff promptly.
• Participates in the resident's care/service plan to improve life quality.
• Assists with mealtimes and feeds residents needing full assistance.
• Fosters relationships with residents and families.
• Supports resident involvement in daily activities and decisions.
• Assists with therapies and treatments under licensed personnel.
• Maintains privacy and HIPAA compliance; knocks before entering.
• Follows safety procedures and reports hazardous conditions or incidents.
• Documents vital signs and other health information in EHR.
• Responds to incoming phone calls.
• May serve as administrator designee if needed.
