Job summary

Mosaic Management is seeking a Health Services Manager in Oregon to lead clinical operations in our care community. The role requires active RN licensure, oversight of resident health services, and strong EMR/MAR proficiency.

You will guide staff, ensure regulatory compliance, and collaborate with care teams to maintain high-quality resident care. The ideal candidate has 3+ years of progressive clinical leadership in long-term care settings and a track record of coaching high-performing teams.

Qualifications

• Active RN license in state of employment.
• 3+ years of progressive clinical leadership in assisted living, memory care, or long‑term care.
• Experience with EMRs, MARs, and regulatory compliance.
• Knowledge of state licensing survey processes.

Responsibilities

• Provide clinical leadership and oversight for resident care services.
• Participate in assessments, move-ins, and service plan reviews per Mosaic policy and state regulations.
• Review health records, monitor weights, and respond to changes of condition promptly.
• Maintain updated internal service plans and ensure staff implement care appropriately.
• Document resident updates daily to coordinate care teams.
• Oversee medication management, including verification of orders, MAR transcription, and PRN parameters.
