Job summary

Norton Healthcare seeks a Social Services Coordinator to join the Care Management team. You will assess post-hospital needs and coordinate transitions to least restrictive settings, interfacing with patients and families.

You will coordinate referrals to rehab, extended care facilities, DME, home health and related resources. You will also assess psychosocial needs, handle crisis interventions, and collaborate with a psychiatrist or Protective Services when required, while upholding Norton

Responsibilities

• Assess patient's post-hospital care needs and ensure transition to the least restrictive care environment.
• Refer to acute rehabilitation, extended care facilities, DME, home health and other resources.
• Assess psychosocial needs and continuum of care; include crisis management and referrals.
