Job summary

Center for Life Management (CLM) in New Hampshire seeks an Integrated Care Senior Case Manager to supervise staff, coordinate care plans, and ensure Medicaid-related services are delivered with high quality.

The full-time role focuses on staff supervision, performance reviews, and coordinating home and community-based care for adults and elders, promoting client wellness and continuity of care.

Qualifications

• Bachelor’s degree and/or Associate’s degree required/preferred
• Two years of full-time clinical experience
• Ability to exercise sound clinical judgment in routine, urgent, and emergent situations
• Proficient with Microsoft Office applications (Word, Excel, Publisher, PowerPoint, Outlook, Teams)
• Ability to document effectively in clinical work
• Ability to navigate an electronic health record
• Ability to work as part of a clinical team and be self-directed with minimal supervision
• Ability to acquire and practice new clinical skills as needed
• Strong communication, organization, and multi-tasking skills

Responsibilities

• Train new and seasoned case managers in case management processes, Medicaid benefits and care planning
• Provide formal supervision to assigned staff including case reviews and productivity reports
• Provide back-up supervision to other case managers
• Manage time off requests and weekly payroll for designated staff
• Develop methods to meet client outcomes, productivity, and budget variances
• Review productivity and problem-solve with staff to meet expectations
• Provide ongoing feedback and performance reviews
• Provide 10 case management contacts per month; incentive pay eligibility
• Provide 4 hours of billable service per week to meet KPI; incentive opportunities
• Participate in leadership and team meetings
• Promote a positive work environment and cooperative culture
• Ensure care planning with treatment objectives and outcomes
• Assist clients with applying for and maintaining medical coverage
• Provide outreach, coordination, resource identification and monitoring of supports
• Offer counseling and Functional Support Services in home or community settings
• Provide Crisis Intervention when needed
• Collaborate with treatment teams for wrap-around care
• Represent CLM professionally in all settings
• Perform other duties as assigned
