Job summary

Professional Case Management (PCM) is hiring a Senior Case Manager in Tennessee to lead in-home nursing care and case management for former nuclear workers. You will supervise RN Case Managers, plan care, and collaborate with regional leadership to ensure high-quality service in the home setting.

The role requires a BSN preferred, active RN license in the state, and at least two years of nursing experience including home care; supervisory experience is a plus.

Qualifications

RN license current and unrestricted
Minimum 2 years nursing experience including 1 year in home care
1 year supervisory/case management experience preferred
CPR certification current

Responsibilities

Provide direct care and case management of assigned clients
Lead and support RN Case Managers and care teams
Coordinate client care conferences and documentation
Ensure adherence to agency policies and nursing scope of practice
Collaborate with regional director to improve services
Assist staffing with scheduling and resource allocation
Document and review clinical records for accuracy
Participate in quality improvement initiatives
