Job summary

Comagine Health is seeking a Senior Improvement Advisor to lead the Utah Community Care Hub initiative, coordinating care transitions and partnerships and driving population health improvements.

You will support partner onboarding, develop workflows, and deliver training while collaborating with health departments, providers, and community organizations to scale the model across rural Utah.

Qualifications

• Bachelor’s degree in health care, public health, or related field.
• 5+ years in health care, public health, or similar fields.
• 3+ years of project management experience.
• Experience leading multidisciplinary teams and complex implementations.
• Strong communication and stakeholder engagement skills.
• Ability to work independently and remotely.

Responsibilities

• Lead implementation activities, readiness assessments, onboarding, workflow development.
• Support local health departments in role development within the Community Care Hub.
• Provide technical assistance on staffing, care coordination, and operations.
• Identify challenges and develop practical solutions with partners.
• Develop and deliver training and educational resources.
• Evaluate progress and expand hub model to rural/tribal communities.
