Comagine Health is seeking a Senior Improvement Advisor to support Utah's Community Care Hub, focusing on coordination between health care providers, local health departments, and community organizations. You will lead readiness, onboarding, and workflow development while ensuring scalable, sustainable solutions.

The role emphasizes training, ongoing improvement, and collaboration across a remote team, with travel within Utah as needed to support partner engagement.

Qualifications

• Bachelor’s degree in health care, public health, community health, or related field.
• Master’s degree in public health, health care administration, or related field preferred.
• Strong communication, facilitation, and stakeholder engagement skills.
• Experience coordinating across multiple partners and remote teams.

Responsibilities

• Lead Community Care Hub implementation and readiness activities.
• Develop referral and care coordination workflows with partners.
• Deliver training and quality improvement resources.
• Engage with health departments, CHWs, and community organizations.
• Support pilot implementation and expansion to rural/tribal communities.
