Comagine Health is seeking a Senior Improvement Advisor to support Utah's Community Care Hub, focusing on care coordination, partnerships, and population health improvements across rural Utah. You will work with providers, local health departments, Community Health Workers, and community organizations to build sustainable systems that connect people with needed services.

Qualifikationen

• Bachelor's degree in health care, public health, community health, or a related field.
• Five or more years in health care, public health, community-based services, quality improvement, or related field.
• Three or more years of project management experience.
• Experience leading or supporting multidisciplinary teams on complex initiatives.
• Strong communication, facilitation, relationship-building, and stakeholder engagement skills.

Aufgaben

• Lead implementation activities for Utah's Community Care Hub, including readiness assessments and partner onboarding.
• Develop referral and care coordination workflows with local health departments and partners.
• Provide training, technical assistance, and QA to participating organizations.
• Identify challenges and collaborate on practical solutions to improve outcomes.
• Engage partners and coordinate plans, training, and milestones across organizations.
• Support pilot implementation, evaluate progress, and expand the model to rural and tribal communities.
