Comagine Health is seeking a Senior Improvement Advisor to support Utah's Community Care Hub, focusing on care coordination, community partnerships, and population health outcomes in rural Utah.

You will lead partner readiness, onboarding, workflow development, and training, while applying quality improvement methods to enhance care access and strengthen clinical-community linkages across the state. This role is remote with Utah focus.

Qualifications

• Bachelor's degree in a healthcare or related field is required.
• Five+ years in health care, public health, or related fields.
• Three+ years in project management.
• Experience leading multidisciplinary teams and complex initiatives.
• Strong communication and stakeholder engagement skills.
• Ability to work independently and remotely.

Responsabilités

• Lead implementation activities, onboarding, workflow development, and ongoing support.
• Develop referral and care coordination workflows for Utah's Community Care Hub.
• Deliver training, technical assistance, and quality improvement resources.
• Engage partners and coordinate meetings, milestones, and governance activities.
• Support pilot implementation and expansion to rural/tribal communities.
