This on-site role delivers wellness services and counseling to residents, goal-setting, and collaborative care planning to help tenants age in place with dignity. Qualifications

• Bachelor's degree in human services, Sociology, Psychology, Social Work or related field
• Two years' experience working in housing, mental health, senior and/or elderly populations or related fields
• Excellent organizational, verbal, & written communication skills
• Ability to work independently as well as part of a team
• Bi-Lingual Chinese/Korean/Spanish/English required
• Regularly required to talk, hear, walk, stand, & sit
• Able to operate a computer keyboard, mouse & office equipment
• Ability to work flexible hours (evenings/weekends) as assigned
• Able to travel throughout the five boroughs as needed
• Ability to lift up to 10lbs

Responsibilities

• Conduct and prepare assessments, case logs, documents case notes, care plans for tenants.
• Establish and maintain contacts with families, social service support agencies, and health facilities.
• Assesses residents for their needs and develops and implements service plans.
• Provide counseling, behavioral interventions, restorative services, assistance in negotiating community activities and crisis management services.
• Assist residents in developing meaningful service plan goals and provide supports to assist residents in achieving their goals.
• Develops wellness goals and activities with input of residents and other staff.
• Ensure the development of appropriate wellness, educational and social programming is consistently taking place.
• Coordinates care for targeted critical/high need resident cases.
• Assist Licensed Practical Nurse with medication management.
• Work through team approach in partnership with building property management and maintenance staff to identify and respond to residents in need.
• Respond to emergency situations on site and provide crisis intervention.
• Maintain contact and follow-up with service providers put in place for residents such as Adult Protective Services and/or outpatient treatment services.
• Strictly adhere to and enforce all the Agency policies regarding Confidentiality and Corporate Compliance regulations and maintain compliance with HIPAA regulations.
• Accompany residents to face to face meetings as requested by the resident.
• Attend staff meetings, agency meetings, workshops, classes, & in-service trainings on a regular basis to maintain current information on eligibility requirements for all basic entitlements.
