Job summary

THRIVE in New York is looking for a case/care manager to help high-risk individuals achieve independence and stability in their psychiatric or medical conditions. The role centers on coordinating medical, social, housing and safety services using a holistic approach that respects culture, education and employment factors, and helps clients access community resources for stabilization and integration.

Qualifications

• BA/BS and two years of relevant experience.
• RN with two years of relevant experience.
• Masters degree with one year of relevant experience.

Responsibilities

• Meets with each service recipient to provide one-on-one support.
• Maintains relationship with the client’s family.
• Reports to supervisor on illnesses, accidents, or events.
• Coordinates all services the person will receive during care management.
• Develops written care management plan based on assessed needs.
• Links client to services listed in the plan.
• Advocates to access needed services in the least restrictive setting.
• Maintains progress notes per agency guidelines.
• Develops and maintains community/provider relationships.
• Assists with academic, rehabilitation, social, employment and health supports.
• Coordinates treatment with other agencies; gathers input for care plan.
• Encourages participation in the care management plan.
• Maintains resident records compliant with policies and audits.
