Job summary

Encore Community Services in New York, NY seeks a Residential Case Manager for Encore 49 Residence. This hybrid, full-time role focuses on case management and rehabilitative services to support housing stability and independence for older adults.

You will conduct assessments, coordinate resources, and collaborate with multidisciplinary teams to prevent hospitalization and homelessness while improving tenants’ quality of life.

Qualifications

• Two years’ experience in social services with a BA in a related field.
• Proven experience with the senior homelessness and/or mental health populations.
• Strong clinical, written, and interpersonal communication skills.
• Proficient in Microsoft Office and cloud-based applications.

Responsibilities

• Provide case management and rehabilitative mental health services to Encore 49 residents.
• Conduct needs-based assessments and connect residents to income resources.
• Maintain documentation and reporting in data entry systems and funding sources’ files.
• Coordinate services with the Senior Center, Meals on Wheels, Adult Day Treatment Programs and other providers.
• Escort clients to hospital visits and accompany them to appointments as needed.
