Qualifications

• MSW from CSWE-accredited program or equivalent recognized for NY licensure.
• Current NY LMSW or LCSW licensure required.
• Knowledge of psychosocial assessment, counseling, and care coordination.
• Strong communication and teamwork skills for multidisciplinary care.
• Ability to document accurately in the electronic medical record.

Responsibilities

• Conduct psychosocial assessments to identify factors affecting health, treatment, and discharge planning.
• Develop and evaluate individualized psychosocial intervention plans with the care team.
• Provide supportive counseling, crisis intervention, and bereavement support.
• Participate in rounds, huddles, and case conferences for discharge needs.
• Coordinate with physicians, nurses, case managers, and community resources.
• Assist with safe, timely discharge plans including home care and DME.
