Job summary

Riverside Community Hospital is seeking a Manager of Case Management to oversee patient-centered coordination across the case management team and ensure alignment with hospital goals. You will supervise staff, ensure compliant practices, and drive improvements in patient throughput and resource utilization.

The role reports to the Case Management Director and requires leadership in line with hospital policies and regulations.

Qualifications

• Bachelor's in Nursing OR Masters in Social Work is required.
• 5 years of case management in an acute hospital setting is required.
• Thorough knowledge of Case Management defined by federal and non-federal programs.

Responsibilities

• Promote patient-centered care by coordinating hospital-based case management in area of responsibility.
• Provide day-to-day oversight and management of the case management program.
• Serve as a resource, advisor and leader for the case management team.
• Supervise staff and ensure effective care coordination and practices.
• Evaluate quality of interactions and improve patient throughput while balancing care and resources.
