BayCare's St. Joseph's Hospital in Tampa seeks a Manager Hospital Care Coordination to lead the department and coordinate care across the continuum. The role focuses on improving patient flow, safeguarding quality, and aligning programs across LVAD, Transplant, and LifeLink services.

Qualifications

• Requires RN licensure or healthcare management leadership background.
• Experience leading acute care, case management, or social services with at least 3 years in a leadership role.
• Must meet BayCare qualifications for case management and coordination leadership.

Responsibilities

• Oversees department team members and coordinates care across the continuum.
• Monitors length of stay and identifies opportunities to reduce LOS through case management.
• Collaborates with LVAD, Transplant, and LifeLink programs to ensure compliant, timely transitions of care.
• Drives disease/care management processes with evidence-based protocols and outcomes.
• Formalizes care coordination roles, policies and procedures; updates as improvements are implemented.
• Partners with ancillary departments and community agencies to improve efficiency and patient outcomes.
