Zusammenfassung

Continuum Hospice is seeking a Registered Nurse with strong clinical knowledge and supervisory experience to serve as Clinical Director, leading an amazing team of clinicians. A $15,000 sign-on bonus is available for eligible candidates.

Schedule: Monday–Friday, 8 am–4:30 pm; Territory: Braintree. You will collaborate with the Program Director, Hospice Physician, and Community Liaisons to ensure excellent patient care, regulatory compliance, and staff development within a mission-driven

Qualifikationen

• Bachelor's degree in nursing or health care preferred.
• RN with a current state license to practice.
• 3+ years nursing experience including 1 year hospice/home health/oncology and 1 year supervisory experience preferred.
• Ability to manage terminal care with crisis intervention and symptom control.
• Proficient in EMR; CPR certification may be required.

Aufgaben

• Coordinate and oversee patient services aligned with hospice program policies.
• Guide and support interdisciplinary team of clinicians.
• Conduct chart audits to ensure regulatory and quality compliance.
• Develop orientation and in-service training for nursing and clinical staff.
• Evaluate and uplift clinical staff performance during probation and annually.
• Lead interdisciplinary team meetings to refine Care Plans and support families.
