Job summary

Bristol Hospice in Colorado is seeking a Clinical Supervisor to ensure care is delivered in accordance with our patient-centered mission. You will supervise clinical staff, coordinate home visits, and work with the Medical Director to review patient eligibility and care plans.

Ideal candidates are licensed RNs with hospice home care experience and two years of supervisory responsibility, strong communication skills, and a dedication to compassionate end-of-life care.

Qualifications

• RN license in good standing required.
• Minimum of 2 years of supervisory or management experience in hospice/home care.

Responsibilities

• Receive case referrals and coordinate home visits to determine care needs.
• Supervise clinical staff and confer with Medical Director regarding patient eligibility and care plans.
• Review case records and provide guidance to clinicians to improve quality of care.
• Assist in orientation and ongoing education for new staff.
• Support Patient Care Services leadership in policy development and quality improvement.

Education

Registered Nurse (RN) license in good standing
