Job summary

Heartland Hospice is seeking a Patient Care Manager (RN) to lead clinical operations, coordinate care, and mentor staff while ensuring compliance with regulatory standards.

Reporting to the Executive Director, this role oversees patient care, triages referrals, leads IDG meetings, and drives performance improvement in a home-based hospice setting.

BSN preferred; 3+ years direct care incl hospice/home care; strong leadership and EMR proficiency required.

Qualifications

• Hold an active RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum 3 years direct patient care, including 2 years in hospice/home-based care.
• Demonstrated knowledge of hospice principles, regulatory standards (Medicare/Medicaid/JCAHO/ACHC).
• Compassion for terminally ill patients and families.
• Ability to lead a diverse care team under emotional/clinical stress.
• Strong interpersonal, organizational, and leadership skills.
• Proficient in clinical documentation and technology systems.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and triage referrals based on clinical judgment.
• Triage incoming referrals and medical concerns promptly and compassionately.
• Lead or facilitate IDG meetings, present updates, and document minutes.
• Mentor and train associates to support development, performance, and compliance.
• Ensure care delivery meets regulatory standards and company policies.
• Support performance improvement, quality assessments, and operational efficiencies.
