Qualifications

• Bachelor's degree or diploma/Associate's degree in Nursing required.
• BSN preferred.
• 5-7 years RN experience (ICU/CCU/Med-Surg/Oncology/Gerontology) within last 5 years required.
• 1-2 years Home health nursing experience preferred.
• RN license in DC/MD/VA or applicable state required.
• Valid driver's license and reliable vehicle required.
• CPR certification required within 90 days.
• Additional unit certifications may vary by department.

Responsibilities

• Contributes to goals and adheres to policies, completes mandatory training.
• Preceptor for field training for new nurses and home health aides.
• Coordinate and evaluate patient care with physicians and team; act as case manager for caseload.
• Ensure Oasis documentation accuracy and regulatory compliance.
• Delegate cases to LPNs and develop individualized care plans with patient/family involvement.
• Develop and deliver teaching plans tailored to patient needs and knowledge.
