Job summary

The role collaborates with physicians, interdisciplinary teams, payers and community resources to coordinate patient care, serving as a case manager for a designated caseload, and documenting in Oasis while delegating to LPNs.

Qualifications

• IV therapy skills.
• Demonstrated comprehensive knowledge of home care case management and OASIS documentation.
• Strong organizational skills.
• Excellent verbal and written communication skills.
• Basic computer skills preferred.
• Smart Phone experience.

Responsibilities

• Collaborates with physicians interdisciplinary team payers and community resources to coordinate patient care and ensure timely communication.
• Serves as a case manager for a designated caseload, prioritizing and managing patient visits and care to meet productivity and quality standards.
• Maintains accurate clinical documentation, records, reports and certifications per agency standards.
• Delegates cases to LPNs and develops individualized care plans with patient and family.
• Uses advanced nursing judgement to implement therapeutic regimens and utilize community resources.
