Qualifications

• RN licensure in DC/MD/VA or state as required
• BSN preferred
• Bachelor's degree or diploma in nursing required

Responsibilities

• Preceptor for field training of new nurses and home health aides.
• Coordinate care with physicians, interdisciplinary team, payers, and community resources.
• Serve as case manager for designated caseload, prioritizing and managing patient assignments, visit schedules and care.
• Ensure Oasis documentation accuracy and regulatory compliance.
• Develop and document individualized care plans with patient/family involvement.
• Delegate cases to LPNs and guide clinical staff.
• Provide patient and family education and evaluate learning impact.
• Participate in quality and service-improvement initiatives.
