MedStar Health in Washington, DC is seeking a dedicated RN Case Manager to coordinate patient care across the care team and document progress. You will assess needs, develop individualized plans, and educate patients and families while collaborating with physicians, insurers, and multidisciplinary staff to optimize outcomes.

Responsibilities include independent caseload management, timely documentation, and adherence to department policies and safety standards.

Qualifications

• Active RN license in DC, MD, or VA; required based on work location
• 1–2 years RN experience (ICU/CCU/Med-Surg/Oncology/Gerontology) within last 5 years
• CPR certification (AHA or Red Cross) within 90 days
• Basic teaching and patient education experience
• Ability to manage a caseload independently and document care plans

Responsabilités

• Analyze and document patient/family responses to interventions and revise care plans
• Collaborate with physicians and staff; communicate with case managers and insurers
• Schedule visits and inform patient/family and supervisor of care plans
• Develop individualized care plans with measurable outcomes and teaching plans
• Identify learning needs and develop teaching plans tailored to patient level
• Maintain clinical records and submit admissions and daily reports timely

Formation

Diploma in Nursing

Associate's degree in Nursing
