Responsibilities

• Directs nursing and home care as prescribed by the attending physician, ensuring accuracy and completeness of plans of care.
• Provides leadership and supervision to the team to deliver compassionate, comprehensive case management.
• Maintains accurate records of all services rendered to patients and evaluates care needs regularly.
• Performs biweekly clinical supervisory visits for Home Health Aide patients and conducts initial admission assessments.
• Develops written plans of care after admission and communicates patient responses to the physician.
