Qualifications

• Bachelor’s degree in nursing from an accredited program and Kentucky RN licensure with at least two years of nursing practice experience in home care and one year supervisory experience.
• CPR certification and TB skin test or willingness to receive.

Responsibilities

• Directs nursing and home care as prescribed by the attending physician.
• Reviews all plans of treatments for accuracy, appropriateness, and completeness.
• Provides leadership, guidance, and supervision to your team, ensuring compassionate and respectful care and comprehensive case management services.
• Maintains accurate records of all services rendered to patients.
• Evaluates and reevaluates the care and nursing needs of each patient.
• Supervises and trains other nursing services personnel; provides orientation and competency/performance oversight.
• Performs clinical supervisory visits on Home Health Aide patients biweekly.
• Initial admission assessment and evaluation; develops written Plans of Care.
• Reports patient responses to treatments to the physician; notes changes in condition.
• Re-evaluates ongoing need for care; initiates recertification as necessary.
• Teaches, counsels, and guides patients and families regarding nursing care needs.
• Supports and enforces agency policies and confidentiality.
