Job summary

At Home Care of Kentucky is seeking a Clinical Supervisor in Bowling Green, KY to provide direct supervision of patient services personnel and case managers. You will coordinate care services, ensure regulatory compliance, and report to the Director of Patient Care Services.

Responsibilities include directing nursing and home care, reviewing treatment plans, supervising staff, and maintaining patient records.

Qualifications

• Bachelor’s degree in nursing from an accredited program.
• Current RN licensure in Kentucky.
• At least two years of nursing practice in home care and one year supervisory experience.
• CPR certification and TB skin test or willingness to receive.

Responsibilities

• Directs nursing and home care as prescribed by the attending physician.
• Reviews all plans of treatments for accuracy, appropriateness, and completeness.
• Provides leadership, guidance, and supervision to the team for compassionate and comprehensive care.
• Maintains accurate records of all services rendered to patients.
• Evaluates and reevaluates nursing needs and patient care requirements.
• Supervises and trains nursing staff, provides orientation and performance oversight.
• Performs supervisory visits for Home Health Aide patients every two weeks.
• Assesses admissions and develops written Plans of Care.
• Reports patient responses to treatments to the physician.
• Recertifies care as needed and teaches care to patients and families.
• Ensures confidentiality and compliance with regulations and agency policies.
• Participates in information management and performance improvement programs.
