Job summary

Concordia Lutheran Ministries is seeking a Clinical Manager (RN/PA-licensed) to supervise home health staff, coordinate staffing, and ensure delivery of quality patient care in alignment with agency policy and regulatory standards.

The role requires Pennsylvania licensure, 2+ years in home health, and strong leadership, communication, and EHR skills. Travel for home visits is expected as needed.

Qualifications

• Registered nurse with current Pennsylvania license; BSN preferred.
• Two years of home health experience with supervisory roles.
• Knowledge of agency policies, federal and state regulations, and third party payers.
• Strong organizational, communication and interpersonal skills; evidence of leadership.

Responsibilities

• Coordinate staffing patterns to meet geographical needs and referrals.
• Maintain staff schedules for daily, weekend and on-call coverage.
• Handle calls and messages to and from staff, patients, families and referrals.
• Ensure clinical staff deliver appropriate services; consult physicians on orders and care plans.
• Interview applicants and assist in hiring to maintain qualified staff.
• Ensure patient needs are continually assessed and care plans updated.
• Provide hands-on care to clinically complex patients to maintain skills.
• Ensure regulatory compliance with local, state and federal standards.
• Participate in Utilization and Quality Improvement activities.
• Identify training needs and assist with in-services.
• Review Plans of Care and sign off before physician signature.
• Supervise all patient care activities for compliance.
• Assist in competency testing for nurses and home health aides.
• Conduct performance reviews of clinical staff.
• Monitor and maintain productivity reports.
• Keep clinical records current and perform random chart reviews.
• Manage the clinical record system including billing aspects.
• Obtain and deliver information to others by phone, fax, reports, letters and in person.
• Coordinate with field staff, patients, families, physicians, insurers, hospitals, labs and government agencies.
• Maintain open door policy to resolve patient and staff concerns.
• Organize staff meetings and represent the agency on clinical matters.
