Qualifications

• PA license required (RN or PT)
• Minimum 2 years home health with supervisory roles
• Knowledge of agency policies, federal/state regulations and third party payers
• Strong organizational, communication and interpersonal skills; leadership evidence
• Reliable transportation with valid driver's license and insurance
• Experience with computers and electronic health record software

Responsabilités

• Coordinates staffing patterns to meet geographical needs and referrals
• Maintains schedules for staff for daily, weekend and on-call coverage
• Handles calls/messages to/from staff, patients, families and referrals
• Accountable for clinical staff and provision of services to patients
• Participates in hiring decisions to maintain qualified staff
• Ensures patient needs are assessed and care plans developed with physicians
• Maintains clinical skills by hands-on care of complex patients
• Works with Compliance and Ethics Manager for regulatory compliance
• Participates in Utilization and Quality Improvement activities
• Identifies in-services needed for staff and assists with them
• Provides back-up to Clinical Supervisor for Plans of Care review and 485 signing
• Back-up support to Clinical Director as needed
• Supports supervision of patient care activities to meet nursing standards
• Assists with competency testing for nurses and home health aides
• Performs performance reviews of clinical staff
• Monitors productivity and maintains reports
