Job summary

Care Advantage Inc. in Annandale, VA seeks a dynamic Registered Nurse (Home Care Supervisor) to lead our Personal Care Team. This full-time role focuses on patient care, team supervision, and ensuring regulatory compliance across community settings.

The nurse will oversee caregivers, guide care plans, and participate in quality improvement while traveling within the service area. A compassionate leader with strong communication skills is required.

Qualifications

• Active RN license in Virginia (compact/multi-state accepted).
• Minimum of 1 year in a nursing role (LPN-to-RN pathway acceptable).
• Associate's Degree in Nursing or higher.
• Strong communication and presentation skills.
• Ability to work collaboratively across teams.
• Reliable transportation for home visits (mileage reimbursed).
• Comfortable with technology (computer/iPad documentation).
• Willingness to travel regularly within assigned area.

Responsibilities

• Provide patient care and advocacy: onboard new patients, explain services, and complete start-of-care assessments.
• Develop individualized care plans addressing medical needs and family involvement.
• Monitor patient progress and adjust care plans; report changes to physicians and care teams.
• Teach or supervise household members assisting with patient care for safety at home.
