Job summary

Rochester Regional Health seeks an experienced Senior Health Home Care Manager to coordinate comprehensive health home services for clients with complex chronic conditions. You will develop care plans, link to medical, behavioral and social services, and support transitions between care levels.

The role involves supervising team members, providing clinical consultation, and collaborating with families and other providers. Requires advanced degree and relevant certification in health services.

Qualifications

Strong analytical, computer, and communication skills.
Master's degree in a qualifying field with experience in health home care management or related area.
Bachelor's degree in a qualifying field with required experience or CASAC with experience.
Licensure in NYS Driver's License is required.
Experience providing direct services to people with SMI or related conditions and linking to services.

Responsibilities

Create, manage and coordinate enrollee care plans across medical, behavioral and social services.
Conduct community outreach and move clients to enrolled status.
Complete documentation within established timeframes.
Caseload includes high-acuity clients and AOT cases; prepare weekly reports as required.
Provide supervision and act as a consultant to team members as delegated.
