• Create, manage and coordinate enrollee care plans across medical/behavioral and social services.
• Complete all necessary documentation within established timeframes.
• Perform and document progressive community outreach with assigned cases and move to enrolled status as appropriate.
• Caseload includes mid-acuity and AOT clients; complete weekly reports per AOT guidelines.
• Performs other duties as assigned
