Wadena County seeks a skilled social worker to handle cases with complex social problems and develop treatment plans. You will manage care coordination for clients across Alternative Care, Elderly Waiver, Nursing Home and MSHP/MSCM services, conducting regular client visits and maintaining thorough documentation.

The role requires strong knowledge of social casework, interviewing, and community resources, with emphasis on collaboration with care teams to prevent unnecessary institutionalization.

Qualifications

• Bachelor’s degree in Social Work or other approved social services degree.
• Valid MN driver’s license required.

Responsabilités

• Provide social work services on cases with complex social problems and treatment plans.
• Perform case management/care coordination for clients in Alternative Care, Elderly Waiver, Nursing Home and/or MSHO/MSCM services.
• Conduct face-to-face visits with clients at least every six months or as health plan requires.
• Maintain client files in Appxtender and SSIS.
• Complete annual assessments, support plans, and service agreements for waivered clients; annual HRA and support plan for community well services; nursing home assessments.
• Collaborate with Care Coordination Team and assist in navigation of healthcare/waiver programs.
