Wadena County Human Services in Wadena, MN seeks an experienced Social Worker to provide complex case management and care coordination for clients receiving Alternative Care, Elderly Waiver, Nursing Home and MSHO/MSC services.

The successful candidate will conduct face-to-face visits, complete annual assessments and service plans, maintain documentation in SSIS, and help clients access community-based services to prevent unnecessary institutionalization.

Qualifications

• Bachelor’s degree in Social Work or approved social services degree.
• Valid Minnesota driver’s license required.
• Experience with case management and care coordination in social services.
• Ability to conduct assessments and develop service plans.
• Strong written and oral communication skills.
• Knowledge of DHS waivers and health plan requirements.

Responsabilités

• Perform case management and care coordination for waiver and MSHO/MSC services.
• Conduct face-to-face visits at least every six months; health plan requirements enforced.
• Maintain client files and documentation in SSIS and related systems.
• Complete annual assessments and service plans for waivered and community well clients.
• Develop community support plans per DHS Person-Centered Protocol.
• Coordinate with Care Coordination Team and DHS guidelines.
• Attend trainings, agency meetings, and contribute to reporting.
• Support public relations efforts when needed.

Formation
