Job summary

UC Davis Health seeks a Patient Navigator IV (Gastroenterology, Hepatology & Nutrition) to lead specialized GI patient care coordination. The role requires autonomous clinical expertise to educate on GI therapies, coordinate nutrition and care plans, and mentor staff while driving quality improvement across programs.

Responsibilities include identifying barriers to care, refining workflows, and ensuring patient-centered communication.

Qualifications

• RN license required and unrestricted
• BLS/ACLS/ACLS certification as applicable
• Bachelor's degree in related area or equivalent experience
• Graduation from an accredited school of nursing
• Minimum 3 years RN experience with ambulatory care exposure
• Knowledge of GI and liver diseases management and patient education

Responsibilities

• Coordinate specialized care plans for GI–hepato-nutrition programs
• Provide clinical education on GI specialty care and biologic therapies
• Mentor and coach clinical team members and standardize workflows
• Lead continuous quality improvement projects within GI/nutrition programs
• Develop and monitor metrics to improve patient outcomes
